- Visit signup.formed.org or Click Here
- Choose Find Your Parish Subscription
- Click on “Continue to Sign up/Login”
- If you are prompted to log in, please follow those prompts. (You may need to check your spam/junk email folder for the sign-in code email.) Then you will be directed to the Find Your Parish page.
- Enter your email and name, click the box to agree to the Terms of Use, and click “Sign Up”
- Select your parish by entering “49504″ or “Holy Spirit Church”, then click “Sign Up”
- You should then be taken to the main Formed site, and should be automatically signed in and be able to access all the content.
